摘要:目的 探讨术前血清C-反应蛋白(C-reactive protein, CRP) 水平在预测非小细胞肺癌(non-small-cell lung cancer, NSCLC)患者预后中的作用。方
法 检测120例NSCLC患者术前CRP水平,以CRP浓度5mg/L为分界点分为CRP正常组和CRP升高组,分析术前不同CRP水平与临床病理特征及预后之间的关系。结果CRP水平升高组较CRP正常组TNM分期更晚、淋巴结转移阳性率更高,差异均具有统计学意义(P<0.05),2组间年龄、性别、病理类型分布均无显著性差异(P>0.05)。单因素及多因素生存分析均显示,TNM分期晚、淋巴结转移阳性及术前CRP升高是预后危险因素。血清CRP水平升高者和血清CRP正常者术后5年生存率分别为31.8%和56.6%(P<0. 001),差异有统计学意义。CRP升高组和正常组的预后风险比(hazard ratio,HR)为2.892,95%,CI为1.984~5.096,P<0.001。结论 NSCLC患者术前血清CRP水平升高是预后的独立危险因素。

Abstract

Abstract: Objective To explore the role of pre-operation serum C-reactive protein (CRP) levels on the prognosis evaluation in patients with non–small cell lung cancer (NSCLC). Methods The preoperative CRP serum levels in 120 patients with NSCLC were measured. The patients were divided into two groups based on the CRP levels: normal CRP group (CRP<5mg/L, n=76) and raised CRP group (CRP≥5mg/L, n=44). The differences of clinical pathological status and long-term prognosis between two groups were analyzed. Results The pre-operation serum CRP levels were significantly associated with advanced tumor TNM stage and lymph node involvement (P<0.05), but not with patients’ age, sex and histological type (P>0.05). In an unvaried and multivariate analysis, tumor TNM stage, lymph node involvement and serum CRP levels were associated with survival rate. The 5-year survival rate of patients with CRP<5mg/L and ≥5mg/L were 56.6% and 31.8% respectively (P<0.001). The patients with CRP≥5mg/L had a hazard ratio (HR) of 2.892 (95%CI: 1.984-5.096, P<0.001). Conclusion Pre-operation serum CRP levels might be used as an additional prognostic evaluation parameter in patients with NSCLC.
