结核性胸膜炎患者腺苷脱氨酶与总蛋白、乳酸脱氢酶及其比值的相关性分析

摘要

目的 探讨胸水与血清中的腺苷脱氨酶(ADA)与总蛋白(TP)、乳酸脱氢酶(LDH)及其比值在结核性胸膜炎诊断中的相关性。方法 随机抽取2014年上半年本院收治的结核性胸膜炎患者50例(结核组),肝性胸水(对照组)20例,进行胸水和血清的ADA、TP、LDH检测,计算其相应的比值,分析胸水和血清中ADA、LDH和其比值之间的相关性。结果 结核组中ADA、TP、LDH含量及其比值明显高于对照组,差异具有统计学意义(P<0.05)。结核组的胸水ADA与TP、LDH呈直线正相关(P<0.01);血清ADA与TP呈直线正相关(P<0.01),与LDH无相关(P>0.05);胸水ADA与血清ADA的比值(PADA/SADA)与胸水乳酸脱氢酶与血清乳酸脱氢酶的比值(PLDH/SLDH)呈正相关(P<0.01),与胸水总蛋白与血清总蛋白比值(PTP/STP)无相关(P>0.05)。结论 胸水与血清中的ADA、TP、LDH及其比值在结核性胸膜炎诊断中具有重要临床意义。

Abstract

Objective   To investigate the correlation of pleural effusion and serum levels and the ratio of adenosine deaminase (ADA), total protein (TP) and lactate dehydrogenase (LDH) in the diagnosis of tuberculous pleurisy. Methods    Fifty cases of tuberculous pleurisy patients (tuberculosis group) and twenty cases of hepatic hydrothorax (control group) were randomly selected and involved in this study. The ADA, total protein (TP), LDH in pleural effusion and serum were detected and the corresponding ratios were calculated. Results   The levels of ADA, TP and LDH in tuberculosis group and their ratios of the pleural fluid to serum were significantly higher than that of control group (P<0.05).  The pleural fluid ADA levels were linear positive correlated with TP and LDH levels in tuberculosis group (P < 0.01). The serum ADA was linear positive correlated with TP (P < 0.01), and no correlation with LDH in tuberculosis group (P > 0.05). The ratio of ADA in pleural effusion and serum levels was positively correlated with the ratio of LDH (P < 0.01), but no correlation with the ratio of TP (P > 0.05). Conclusion The ratio of ADA, TP and LDH in the pleural effusion and serum has the important clinical significance in diagnosis of tuberculous pleurisy.
