目的 研究妊娠期亚临床甲状腺功能减退（SCH）合并妊娠期糖尿病（GDM）的孕期并发症及妊娠结局。方法 选择140例妊娠合并SCH及122例甲状腺功能正常孕妇，常规进行妊娠期糖尿病筛查，分成SCH合并GDM组、SCH组、GDM组、正常组，观察记录4组孕妇孕期情况及妊娠结局。结果 妊娠合并SCH组的自然流产率及妊娠期糖尿病的发生率明显高于甲功正常组；SCH合并GDM组孕期胎儿生长受限、羊水过少、早产的发生率明显高于甲功正常组，且妊娠结局中胎儿窘迫、剖宫产、新生儿窒息的发生率亦明显高于甲功正常组，差异均有统计学意义（P<0.05）。结论 妊娠期SCH合并GDM对母儿造成的危害较大，应该尽早筛查进行治疗。

Objective To explore the effect of subclinical thyroid function in pregnancy loss (SCH) complicated with gestational diabetes mellitus (GDM) during pregnancy complications on pregnancy outcome. Methods 140 cases of pregnancy complicated with SCH and 122 cases of normal thyroid function of pregnant women were selected for routine screening of gestational diabetes mellitus. The patients were then divided into SCH with GDM group, SCH group, GDM group, normal group. The outcome of 4 groups of pregnant women during pregnancy were observed and recorded. Results The rate of spontaneous abortion and the incidence of diabetes in pregnancy with SCH group were significantly higher than that of normal thyroid function group. The fetal growth restriction, oligohydramnios, premature delivery of SCH combined with GDM group was significantly higher than that in normal thyroid function group, and the outcome of pregnancy in fetal distress and caesarean section and the incidence of neonatal asphyxia were obviously higher than that in normal thyroid function group (P<0.05). Conclusion The subclinical hypothyroidism with gestational diabetes mellitus in pregnancy women during pregnancy may have great harmful on mothers and infants, and should be screening early for treatment.
