目的 探讨D-二聚体在急性缺血性脑卒中(AIS)的变化及诊断价值。方法 检测78名对照组健康人群和173例AIS组患者血浆D-二聚体含量。结果 AIS组血浆D-二聚体含量明显高于对照组（P<0.05）；与对照组相比，小面积梗死组、中等面积梗死组和大面积梗死组血浆D-二聚体含量明显升高（P<0.05）；且D-二聚体含量与脑梗死灶面积呈正相关（r=0.926，P<0.05）。结论 AIS患者血浆D-二聚体含量明显升高，且随梗死面积的增大呈上升趋势。血浆D-二聚体对AIS的辅助诊断具有重要意义。

Objective To evaluate the change and diagnosis value of D-dimer in acute ischemic stroke. Methods The serum levels of D-dimer in 78 normal controls and 78 patients with acute ischemic stroke were detected. Results The serum levels of D-dimer in patients with acute ischemic stroke were higher than that in normal controls (p<0.05). Compared with the normal controls, the serum levels of D-dimer in small infarction focus group, middling infarction focus group and large infarction focus group were obviously increased (p<0.05). The serum levels of D-dimer had good linear correlation with the area of infarction focus（r=0.926，p<0.05）. The serum levels of D-dimer in patients with acute ischemic stroke were obviously increased, and showed higher with the increasing of the area of infarction focus. Conclusion The serum levels of D-dimer has an important significance in the auxiliary diagnosis of acute ischemic stroke.
