摘要

目的 建立本地区健康孕妇早中晚孕期的甲状腺激素参考范围,为诊断、治疗、监测孕妇甲状腺疾病提供参考依据。方法 用化学发光免疫测定方法测定健康妊娠孕妇以及非妊娠妇女甲状腺激素水平,其中孕早期137例,孕中期147例,孕晚期139例,非妊娠妇女130例,分别建立甲状腺激素参考值范围。 结果 以中位数(M值)及双侧限值(P2.5和P97.5)表示甲状腺激素在孕早中晚期及非妊娠妇女的参考值范围,孕妇的TSH水平在孕早期明显低于非妊娠妇女(P<0.01),孕中期回升,到孕晚期仍低于非妊娠妇女;孕妇的TT4和TT3自孕早期开始即明显升高(P<0.01),孕中期到最高;孕妇的FT4和FT3随妊娠时间下降,孕中期和孕晚期均明显低于非妊娠妇女(P<0.01)。结论 孕妇的甲状腺激素水平与非妊娠妇女存在明显差异,各孕期之间亦存在明显差异,建立各孕期的甲状腺激素参考范围为临床妊娠期甲状腺疾病的诊治提供了较好的临床价值。

Abstract

Objective To establish the thyroid hormone reference range for healthy pregnant women during different pregnancy period and to provide reference for the diagnosis, treatment, monitoring of maternal thyroid disease. Methods The thyroid hormone levels in healthy pregnant women including 137 cases of early pregnancy, 147 cases of second trimester pregnancy, 139 cases of late pregnant and 130 cases of non pregnant women were determined. Results The TSH level in early pregnancy wonmen was significantly lower than that of non pregnant women (P<0.01), and it increased gradually as the process of pregnancy, but still lower than that of non pregnant women. The serum TT4 and TT3 levels were increased from early pregnancy and reached the highest levels at the second trimesterpregnant (P<0.01). The serum FT4 and FT3 levels in second trimester and late pregnancy were significantly lower than that of non pregnant women (P<0.01). Conclusion The thyroid hormone levels in pregnant women and non pregnant women are different. There are obvious differences between each pregnancy period.The establishment of reference range of thyroid hormone during pregnancy could provide the clinical value for diagnosis and treatment of thyroid disease in pregnancy wonmen.
