目的 评估甲状腺过氧化物酶表达在桥本氏甲状腺炎诊断的价值。方法 通过对中国期刊全文数据库、中国科技期刊全文数据库、万方数据库进行系统检索，检索起止时间为建库之日至2013年10月载有的所有已正式发表的科研文献。检索条件为在期刊数据库的主题中检索包括 “甲状腺过氧化物酶”或“TPOAb”、“桥本氏甲状腺炎”或“桥本氏病”。采用随机效应模型进行数据合并。结果 桥本氏甲状腺炎组与健康对照组血清甲状腺过氧化物酶水平和阳性率差异有统计学意义，桥本氏甲状腺炎组血清甲状腺过氧化物酶水平和阳性率显著高于健康对照组。甲状腺过氧化物酶用于诊断桥本氏甲状腺炎的汇总敏感度为0.89（95% CI：0.87～0.91），汇总特异度为0.96（95% CI：0.95～0.97），SROC AUC为0.9843。结论 甲状腺过氧化物酶用于诊断桥本氏甲状腺炎的敏感度和特异度都较高，具有较高的诊断应用价值。

Objective To evaluate the value of the thyroid autoimmune antibodies (TPOAb) in the diagnosis of Hashimoto's thyroiditis. Methods We searched CNKI, VIP Information, Wanfang data for studies on he TPOAb and Hashimoto's thyroiditis. We also reviewed reference listserv to seek the data missed by our search. Random effects meta-analysis was used to pool data. Results The level of TPOAb and the percentage of TPOAb positive in the Hashimoto's thyroiditis patients were both significantly higher than the control group. The pooled sensitivity and pooled specificity on the thyroid autoimmune antibodies (TPOAb) in the diagnosis of Hashimoto's thyroiditis were 0.89（95% CI：0.87-0.91）and 0.96（95% CI：0.95-0.97）respectively, and the SROC AUC was 0.9843. Conclusion The sensitivity and specificity on the thyroid autoimmune antibodies (TPOAb) in the diagnosis of Hashimoto's thyroiditis are both in a high level. TPOAb might be a good biomarker in diagnosis of Hashimoto's thyroiditis.
