摘要：目的 探讨胸腔积液及血清肿瘤标志物癌胚抗原（CEA）、神经元特异性烯醇化酶（NSE）、细胞角蛋白片段21-1（CYFRA21-1）联合检测在鉴别诊断胸腔积液性质中的意义。方法 采用双抗体夹心磁微粒化学发光法分析42例确诊为肺癌患者和65例确诊为肺炎患者胸水和血清中CEA、NSE及CYFRA21-1浓度，对两组进行比较。结果 肺癌组胸水及血清肿瘤标志物CEA、NSE及CYFRA21-1测定值均显著高于肺炎组，差异有统计学意义，P<0.01。灵敏度和准确度高于单项检测的灵敏度和准确度。结论 胸水和血清3种肿瘤标志物联合检测胸腔积液性质的诊断效能高于单项检测；胸水检测的灵敏度及准确度高于血清学检测。

Abstract: Objective To explore the value of combined detection of CEA, NSE and CYFRA21-1 in differential diagnosis of hydrothorax with serum and hydrothorax. Methods The hydrothorax and serum CEA, NSE and CYFRA21-1 levels in 42 lung cancer and 65 pneumonia patients were detected by double-antibody sandwich chemoluminescence. Results The hydrothorax and serum level of CEA, NSE and CYFRA21-1 in the lung cancer patients were significantly higher than that of pneumonia group (P<0.01). The sensitivity and accuracy of combined CEA, NSE and CYFRA21-1 were higher than each sigle test Conclusion The sensitivity and accuracy of combined detection of CEA, NSE and CYFRA21-1 in serum and hydrothorax are remarkably higher than each single test. The detection of hydrothorax was more valuable than the serum detection.
