目的探讨血清CEA、CA19-9、细胞角质素片段19(CY211)和CK联合检测对结肠息肉和结肠癌的诊断价值。方法检测结肠息肉组和结肠癌组患者及对照组血清CEA、CA19-9、CY211和CK含量。二元Logistic回归分析。结果结肠癌组与结肠息肉组相比较，CEA和CA199含量均显著升高(P＜0.05)，而CK含量显著降低(P＜0.05)。CEA在结肠息肉组和结肠癌组患者的诊断价值最高，其曲线下面积为0.728，诊断灵敏性和特异性为54.9%和91.4%。当CEA、CA199、CY211和CK联合检测时，其曲线下面积为0.847，诊断灵敏性和特异性分别为73.6%和85.5%。结论与单独指标检测相比，联合检测同时兼顾灵敏性和特异性，可作为临床结肠癌的辅助诊断方法。

Objective To evaluate the diagnostic value of combined detection of serum CEA, CA19-9, cell keratin fragment 19 (CY211) and CK to distinguish colonic polyps and colon cancer. Methods The serum levels of CEA, CA19-9, CY211, and CK were detected in the colon polyps and colon cancer,and control group. The binary logistic regression analysis was used for analyze the diagnostic value of combined detection of four markers. Results The serum CEA and CA199 levels in colon cancer were significantly increased than that of colon polyps group (P＜0.05), but the serum CK in colon cancer was significantly decreased(P＜0.05). When the four indicators were used alone to distinguish between colon polyps and colon cancer patients, CEA showed the best diagnostic value in the four indicators, the area under curve was 0.728. The diagnostic sensitivity and specificity with CEA were 54.9% and 91.4% respectively. When combined detection of CEA, CA199, CY211, and CK, the area under curve was 0.847. The diagnostic sensitivity and specificity were 73.6% and 85.5% separately. Conclusion The combined detection of CEA, CA19-9, CY211, and CK could increase the sensitivity and specificity in the clinical diagnosis of colon cancer.
