摘要：目的　探讨甲胎蛋白异质体（AFP-L3）在诊断原发性肝癌中的临床应用价值。方法　收集137例甲胎蛋白阳性患者（>10ng/mL）血清样本，采用亲和吸附法测定AFP-L3含量，计算AFP-L3占总AFP的比例（％）。结果　AFP-L3比值在肝癌组为17.63±5.78，显著高于肝硬化组（8.24±2.49）、肝炎组（7.12±2.01）和对照组（5.49±1.78），差异具有统计学意义（P<0.01）；AFP-L3比值诊断原发性肝癌的ROC曲线下面积分别为0.894。结论 AFP-L3比值可以作为原发性肝癌临床诊断的分子标志物，对临床具有辅助诊断和鉴别诊断价值。

Abstract：Objective To explore the clinical values of alpha-fetoprotein-L3（AFP-L3) in diagnosis of primary liver cancer．Methods The serum samples of 137 patients with elevated AFP level (>10ng/mL）were detected by chemiluminescence immunoassay for AFP and centrifugal column method was used for AFP-L3 measurement．The proportion of AFP-L3/A FP（％）was calculated in the following analyses．Results T serum level of AFP-L3 in primary liver cancer group, liver cirrhosis group, chronic hepatitis B group, and control group were 17.63±5.78%, 8.24±2.49%, 7.12± 2.01% and 5.49±1.78%，respectively．The positive rate of AFP-L3 was significant higher in primary liver cancer group than other groups（P<0.01）．Area under the ROC curves of AFP-L3 was 0.894．Conclusion  AFP-L3could be used as biomarkers in the diagnosis of primary liver cancer, and may also has a clinical value in the auxiliary diagnosis and differential diagnosis．
